CASUAL PARTICIPANT RISK ACKNOWLEDGEMENT, WAIVER & CLASS POLICY

IMPORTANT – PLEASE READ BEFORE SIGNING

Glass blowing, torchwork, kiln fusing, mosaics and other glass art activities involve inherent risks.

Activities at Tamborine Glass Blowing may involve molten glass, open flames, furnaces, kilns, torches, soldering irons, hot and sharp glass, grinders, power tools, sharp tools and other potentially dangerous equipment.

By signing this form, you acknowledge these risks and agree to follow all safety instructions provided by Tamborine Glass Blowing.


PARTICIPANT DECLARATION

This form must be completed and signed by the person participating in the class.

If you are an adult participant, another person, including your partner, friend, family member or another member of your booking group, cannot complete or sign this form on your behalf.

For participants under 16 years of age, this form must be completed by their parent or legal guardian.

By completing and signing this form, I confirm that:

  • The participant information I have provided is true and correct.

  • I am the person named on this form, or I am the parent/legal guardian of the minor named on this form.

  • I have personally read and understood this Risk Acknowledgement, Waiver and Class Policy.

  • I understand the risks associated with participating.

  • I am capable of understanding and following instructions provided by Tamborine Glass Blowing staff.

  • I am voluntarily agreeing to participate.

If an adult participant cannot personally complete and sign this form at the time of their class, they will not be permitted to participate.

If you are unable to personally complete and sign your waiver at the time of your class, no refund, credit, transfer or rescheduling will be provided.


ALCOHOL, DRUGS & INTOXICATION

I understand that I must not attend or participate in any Tamborine Glass Blowing class, workshop or studio activity while intoxicated or impaired by alcohol, recreational drugs or any other substance that may affect my ability to participate safely.

I understand that Tamborine Glass Blowing staff have the right to refuse or immediately discontinue my participation if, in their reasonable opinion, I appear intoxicated, impaired or otherwise unable to safely participate.

Another person cannot enter my details, accept this waiver or sign this form on my behalf.

I understand that if I am refused participation or my participation is discontinued because of intoxication, impairment, unsafe behaviour or an inability to personally complete and understand this waiver:

I will not be entitled to a refund, credit, transfer or rescheduling of my booking.

Where alcohol is permitted as part of a particular event Mosaics or Kiln Fusing, it may only be consumed at designated times and in accordance with instructions from Tamborine Glass Blowing staff.


I ACKNOWLEDGE AND AGREE THAT:

1. Voluntary Participation

I understand that I participate voluntarily and at my own risk in Tamborine Glass Blowing activities.

2. Inherent Risks

I acknowledge that glass art activities may involve:

  • molten glass;

  • furnaces and kilns;

  • open flames and torches;

  • soldering irons;

  • extremely hot materials and surfaces;

  • sharp or broken glass;

  • grinders;

  • power tools;

  • hand tools; and

  • other potentially dangerous equipment and materials.

I understand that these activities are inherently hazardous and may result in burns, cuts, serious injury, property damage or, in extreme circumstances, death.

3. Safety Instructions

I agree to follow all safety instructions and directions provided by Tamborine Glass Blowing staff.

I understand that failure to follow safety instructions may result in my participation being immediately discontinued.

4. Right to Refuse Participation

I understand that Tamborine Glass Blowing reserves the right to refuse or discontinue my participation where staff reasonably believe that I:

  • am intoxicated or impaired;

  • am unable to understand or personally complete this waiver;

  • have had another person complete or sign this waiver on my behalf;

  • am unable to understand or follow safety instructions;

  • am behaving aggressively, recklessly, abusively or inappropriately;

  • am deliberately misusing tools or equipment;

  • am wearing clothing or footwear that creates an unacceptable safety risk; or

  • otherwise present a safety risk to myself, staff, visitors or other participants.

Where participation is refused or discontinued for these reasons, no refund, credit, transfer or rescheduling will be provided.

5. Assumption of Risk

Knowing and understanding the risks involved, I voluntarily request permission to participate, or for my minor child to participate, in Tamborine Glass Blowing activities.

I voluntarily assume the inherent risks associated with participation in these activities.

6. Personal Property

I understand that Tamborine Glass Blowing is not responsible for loss of or damage to my personal property except where liability cannot lawfully be excluded.

7. Minors

Where I am signing for a participant under 16 years of age, I confirm that I am their parent or legal guardian and have authority to provide this acknowledgement on their behalf.

Glass classes and workshops are generally available to participants aged 18 years and over, unless otherwise stated in the individual class description.

Participants under 18 years of age must be accompanied by a responsible paying adult unless prior approval has been provided by Tamborine Glass Blowing.

I understand that Tamborine Glass Blowing activities are not childcare services.

Tamborine Glass Blowing is not responsible for unattended children on the premises before or after their scheduled class or activity.

8. Medical Treatment

Where emergency medical treatment is reasonably required during an activity, I authorise Tamborine Glass Blowing staff to seek appropriate emergency medical assistance.

I understand that I am responsible for my own medical, hospital or other expenses associated with treatment except where otherwise required by law.


DRESS CODE & SAFETY REQUIREMENTS

For your safety, appropriate clothing must be worn.

Footwear

• Closed-toe shoes must be worn.
• Steel-capped footwear is welcome but is not compulsory.

Clothing

• Cotton or other natural-fibre clothing is strongly recommended.
• Long or short-sleeved T-shirts are suitable.
• Loose sleeves and loose-fitting clothing should be avoided.
• Cotton, denim or other natural-fibre pants or shorts are recommended.

Hair

Long or loose hair must be securely tied back.

Personal Protective Equipment

Tamborine Glass Blowing will provide appropriate PPE where required, including safety glasses, heat-resistant gloves and aprons.

I agree to wear PPE when instructed and follow all studio safety requirements.


CANCELLATION & RESCHEDULING

I acknowledge that I have read and agree to Tamborine Glass Blowing’s cancellation and rescheduling policies.

Class and workshop bookings are non-refundable.

A minimum of 14 days’ notice is required to request a reschedule, and applicable rescheduling fees may apply.

No-shows, late cancellations and participants who are refused participation because of intoxication, impairment, unsafe behaviour or failure to complete the required waiver are not eligible for a refund, credit, transfer or rescheduling.


PHOTOGRAPHY & VIDEO

Tamborine Glass Blowing may photograph or record activities within the studio for promotional, archival, website, social media and marketing purposes.

If I do not wish to be photographed or recorded, I understand that I should advise a Tamborine Glass Blowing staff member before my class begins.


FINAL PARTICIPANT DECLARATION

By signing this form, I confirm that I have read, understood and agree to the above Risk Acknowledgement, Waiver and Class Policy.
